You may qualify for the CDCP if all four of these requirements apply to you.
Denture care
The plan may cover several forms of removable denture care when CDCP requirements are met.
Some treatments require approval from the CDCP before treatment begins. The clinic will discuss any known requirements with you.

Your CDCP contribution is based on adjusted family net income and applies to the CDCP-established fee for eligible services.

The CDCP uses its own established fees, which may be different from the clinic's fees. Depending on your coverage and treatment, you may still have an amount to pay. We will help you understand expected costs before treatment begins.
